Bidirectional Cavopulmonary (Glenn) Anastomosis in Pulmonary Atresia with Intact Ventricular Septum and Right Ventricle Dependent Coronary Circulation

چکیده
This is a case report of an 8-month-old infant with congenital heart disease including; pulmonary atresia, intact ventricular septum, right ventricular hypoplasia, atrial septal defect, coronary sinusoids (fistulas between right ventricle and left anterior descending artery). The patient had a stent for patent ductus arteriosus at 3 months of age and after 4 months a bidirectional Glenn shunt operation was performed with successful haemodynamics postoperatively.

When coronary arteries need systolic pressure: surgical considerations.
OBJECTIVES Patients with pulmonary atresia and intact ventricular septum (PA-IVS) may have coronary sinusoids connected to a hypertensive right ventricle. Coronary perfusion may then completely depend on the right ventricular systolic pressure wave; decompression of the right ventricle can be deleterious in these patients. This study was set to investigate the treatment strategies and outcomes ...
متن کاملBidirectional shunting in a coronary artery-right ventricular fistula associated with pulmonary atresia and an intact ventricular septum.
DESCRIPTIONS of anomalous arteriovenous-like communications between the right ventricle and coronary arteries in the presence of pulmonary atresia or severe pulmonary stenosis have been published with increasing frequency since the case report and review of the literature by Williams et al.1 in 1951. The majority of patients with this pathologic complex die in early infancy and are diagnosed at...
متن کاملPulmonary arteriovenous fistulas in patients with left isomerism and cardiac malformations.
OVER THE PAST DECADE, VARIOUS FORMS OF cavopulmonary anastomoses (the bidirectional Glenn shunt, the hemi-Fontan procedure) have become the palliative procedure of choice for the treatment of hearts with a functionally single ventricle. Significantly, the occurrence of pulmonary arteriovenous fistulas had been noted as an important 'late' complication after construction of the classic Glenn ana...
متن کاملPulmonary atresia with intact ventricular septum and ventriculocoronary communications: surgical significance.
The first stage of a repair of pulmonary atresia with intact ventricular septum (type I) was attempted in a 2-day-old infant. At surgery, decompression of the hypertensive small right ventricle was followed by a sudden loss of myocardial contractility and death. Postmortem examination revealed a fistula with a large orifice in the right ventricular infundibulum that communicated directly with t...
متن کاملNew percutaneous techniques for perforating the pulmonary valve in pulmonary atresia with intact ventricular septum.
We report new percutaneous techniques for perforating the pulmonary valve in pulmonary atresia with intact ventricular septum, in 3 newborns who had this birth defect. There was mild to moderate hypoplastic right ventricle, a patent infundibulum, and no coronary-cavitary communications. We succeeded in all cases, and no complications related to the procedure occurred. The new coaxial radiofrequ...
